Forms designed to chart children's development, immunization records and other pertinent medical history can make the record keeping of a pediatric practice move with greater ease. But if the forms aren't used properly they can be just as ineffective as using no forms at all. The problem is more often one of non-use rather than misuse, said Dorothy Davies Johnson, M.D., FAAP. Dr. Johnson, a practicing pediatrician in La Jolla, Calif., noted that forms that require the least amount of writing work best. Pediatricians can fill them out quickly and often fill them out more completely.
